How to Claim

Step 1: Review and Gather Documentation Requirements

If you have received a confirmation email/letter requiring additional documentation to file your claim, the state requires at minimum the signed claim form and photo identification. To determine whether a claim needs additional documentation, refer to the confirmation email/letter sent to you. The items required to approve the claim will be listed on the claim form attachment.
Please note, not all documentation requirements might be listed on the claim form. We will inform you if we require more documentation to process your claim.

Step 2: Submit Claim Documentation

You can use any of the following delivery methods:

  1. Upload your documentation to the state through our secure online portal (opens in a new tab). You can access this portal by hovering over Claiming Property > "Upload Claim Documentation."
  2. Mail in your documentation to the following address:
